Dental insurance

Two coverage options are available, the Dental Base Plan and the Dental Buy Up Plan. Both options cover three classes of reasonable and customary expenses: preventive, basic and major services, and the Buy Up Plan expands coverage to include orthodontics (up to $1,500 maximum lifetime benefit), lower deductible and higher annual maximum.

The dental plan utilizes a passive network, which means you have the ability to receive services from network or non-network providers. No matter your provider, your deductible and coinsurance remain the same as long as charges are reasonable and customary.

? Eligibility

Benefit-eligible faculty and staff may enroll and may cover elgible dependents. 


Coverage for dependents

Dependents may include:

  • Spouse or Sponsored Adult Dependent of an employee
  • A child of an employee who is younger than 26 years of age
  • A child of an employee over the age of 26 who is mentally or physically incapable of self-sustaining employment and meets other plan requirements
  • Child for whom health care coverage is required through a Qualified Medical Child Support Order or other court/administrative order.

Dental Base Plan
  • Self only: $15.86
  • Self and spouse: $31.72
  • Self and child(ren): $38.49
  • Self, spouse and child(ren): $54.35
Dental Buy Up Plan
  • Self only: $28.83
  • Self and spouse: $57.61
  • Self and child(ren): $90.68
  • Self, spouse and child(ren): $121.66
